The novel coronavirus, Coronavirus Disease 2019 (COVID-19), which causes respiratory illness in people, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is contagious and is believed to spread mainly by airborne particulates from person-to-person contact.
After consultation with the Children’s Hospital of Philadelphia, the Philadelphia Boys and Girls Choir (PBGC) will resume its on-site rehearsals beginning the week of September 10, 2022. I acknowledge and understand that the circumstances regarding COVID-19 are changing from day to day and that accordingly, official public guidance is regularly modified and updated, and I accept full responsibility for familiarizing myself, my child, and my family with the most recent updates, as well as the changing conditions in our community, and all state and local orders. I understand that the decision to send my child to PBGC rehearsals or events is entirely voluntary on my part.
I understand that, as PBGC resumes on-site operations, PBGC has put in place some preventative measures to reduce the spread of COVID-19. I am aware of and fully understand that PBGC cannot and does not guarantee, even with the steps it is taking, that my child or myself will not be exposed to COVID-19. By voluntarily choosing to have my child attend a PBGC rehearsal or event, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child or I could be exposed, infected, or re-infected by COVID-19 by attending a choir rehearsal or participating in choir activities, and that such exposure or infection may result in severe illness, permanent disability, or death. I further acknowledge that the risk of becoming exposed to or infected by COVID-19 at PBGC could result from the actions, omissions, or negligence of myself and others, including, but not limited to, other children and their families, PBGC employees, volunteers, or other third parties. I recognize that there is still a risk of contracting the virus, particularly in group settings, even if I and others follow precautions designed to lower the risk of infection.
By allowing my child to attend a PBGC rehearsal or other activity, I knowingly and voluntarily accept and assume the risk of my child of potential exposure to, and infection of, COVID-19, and the associated risks set forth herein. I understand these risks and willingly choose to accept them.
I also understand, agree and acknowledge that I shall not send my child to choir rehearsal or other choir activities or performances if my child is experiencing any symptoms associated with COVID-19, including but not limited to fever, headache, sore throat, shortness of breath, chills, muscle pain, new loss of taste or smell, gastrointestinal symptoms (like nausea, vomiting, or diarrhea), and cough, has been exposed to COVID-19, or anyone in my child’s household(s) has COVID-19 or is symptomatic and is being tested for COVID-19. I will complete the wellness check before dropping off my child at each rehearsal. I understand that if my child is experiencing any symptoms once dropped off, I will be contacted and expected to come and pick up my child.
On behalf of my child and myself, I hereby unconditionally release PBGC, its employees, agents, volunteers, singers, officers and directors from any and all claims arising from any illness or injury that my child may sustain or suffer related to Covid-19 or other virus or infection as a result of attendance at, or travel to or from, any PBGC rehearsal, activity or event, or any related event, including, without limitation, any injury or harm resulting from any testing protocol and or Covid-19 testing or testing results, or any failure of any person to comply with any such protocol and those caused by the negligence and other culpable conduct of the PBGC. I understand and agree that this waiver shall be enforceable and governed by Pennsylvania law and I covenant and agree that I will not pursue any claim against the PBGC arising out of my child’s contracting COVID-19 at any choir rehearsal, performance or other PBGC event and I agree to indemnify, defend and hold the choir harmless should any other person or entity pursue a claim released herein for or on behalf of my child.
